Additionally, accidental insurance coverage of Rs. 2 lakh each will also be provided for the breadwinner and three other family members under CM's Life Insurance Scheme. The accidental insurance component covers both disability and death.

- Firstly visit the Nagaland CM Universal Life Insurance Scheme official website at https://cmlis.nagaland.gov.in/
- At the homepage, click at "Register" tab or directly click https://cmlis.nagaland.gov.in/register
- Upon clicking the link, the page to make CM Universal Life Insurance Scheme registration 2024 will open.
- Here you can "Select Type" as either 'CMHIS Card ID' or 'Ayushman Card ID' or 'Self Registration'.
- For instance, we have selected 'Self Registration' type. It will then ask for mobile number. Enter your mobile no and click "Check Mobile Number" button.
- Next click "Send OTP" button. Then enter OTP received on your mobile number and click "Verify OTP" button.
- After verification of OTP, the Nagaland CM Life Insurance Scheme online registration form 2024 will appear as shown below:-
- Enter the details of the breadwinner such as name, father's name, mother's name, email, mobile number, date of birth, bank account details, address details, nominee date and click at "Add Breadwinner" button.
