Unique ID for Persons with Disabilities (UDID) project is being implemented with a view of creating a National Database for PwDs. Central govt will issue a Unique Disability Identity Card to each person with disabilities.
The UDID project will not only encourage transparency, efficiency and ease of delivering the government benefits to the person with disabilities, but also ensure uniformity. UDID card project will also help in stream-lining the tracking of physical and financial progress of beneficiary at all levels of hierarchy of implementation - from village level, block level, District level , State level and National level.

UDID Card Apply Online (New)
- First of all, go to official website of Unique Disability ID at https://www.swavlambancard.gov.in/
- Then click "Register" link or simply click https://www.swavlambancard.gov.in/pwd/application
- Then UDID card apply online form will open where Persons with Disability registration is required.
- You can hereby enter personal details, disability details, employment details, identity details.
- You will also have to upload certain documents like your photograph, signature, address proof.
- Finally, submit the completed UDID card application form.
Person With Disability Registration Form PDF
To apply for UDID card, one can also adopt offline method. Applicant can download Person with disability registration form PDF - https://www.swavlambancard.gov.in/files/PWD-form-full.pdf. On click that link, PWD registration form will open in PDF format.
Download it on your PC/laptop/smartphone, fill it manually and submit it to complete UDID card registration process.

Once you have successfully created your account i.e after registration process is completed, you can make login at swavlambancard.gov.in portal. In order to make UDID card portal login, you can click at the link - https://www.swavlambancard.gov.in/home/login. Page for unique disability id portal login will open.
Enter enrolment number / UDID number, date of birth and then click "Login" button.You can also edit application (update personal profile) through link - https://www.swavlambancard.gov.in/pwd/editapplication. Updation is only possible after you make UDID card login.
UDID Card Renewal Application Form
- First of all, go to official website of Unique Disability ID at https://www.swavlambancard.gov.in/
- Then click "Apply for Disability Certificate & UDID Card Renewal" link or simply click https://www.swavlambancard.gov.in/pwd/pwdrenewalcard
- Then UDID Card renewal application form will open.
- Enter enrolment number, UDID card number, email ID, mobile number, applicant first name, mother name, father name, date of birth, address, expiry date, disability type.
- Finally, click "Submit" button to complete UDID card renewal.
Apply for Lost UDID Card
- First of all, go to official website of Unique Disability ID at https://www.swavlambancard.gov.in/
- Then click "Apply for Lost UDID Card" link or simply click https://www.swavlambancard.gov.in/pwd/pwdlostcard
- In new page, you can apply for lost UDID card.
- Enter enrolment number, UDID card number, date of birth and click at "Submit" button.
Download e-Disability Card / e-UDID Card
- First of all, go to official website of Unique Disability ID at https://www.swavlambancard.gov.in/
- Then click "Download your e-Disability Card & e-UDID Card" link or simply click https://www.swavlambancard.gov.in/pwd/downloadcertificate
- However, in order to download e UDID card or e Disability Card, you will have to first make login.
- Only after logging it, you can download your certificate.

All States/UTs have started generating UDID cards.Note:-
- Persons with Disabilities (PwDs) have already started applying online for Disability Certificate/UDID Cards from all States/UTs.
- Enrolment /Application Registration number will be generated for all the applications submitted successfully.
- SMS confirmation will be sent to the applicant with the enrolment/application registration number
- Applicants who do not have Disability Certificate will receive SMS with date and time to visit the nearest CMO office for assessment
- The Disability Certificate /UDID Card will be generated online after assessment and verification
- SMS confirmation will be sent to the applicant with the allocation of UDID number
- The status of the application can be checked online by enrolment/application registration number

UDID Card Benefits
The UDID card shall bring a host of benefits to the Persons with Disabilities as given below:- Persons with disabilities will not need to make multiple copies of documents, maintain, and carry multiple documents as the card will capture all the necessary details which can be decoded with the help of a reader.
- UDID card will be the single document of identification, verification of the disabled for availing various benefits in future.
- The UDID Card will also help in stream-lining the tracking of the physical and financial progress of beneficiary at all levels of hierarchy of implementation – from village level, block level, District level, State level and National level.
